In accordance with Ohio state guidelines, all students are required to have a current physical examination form on file before attending school each year. This requirement helps ensure the health and safety of every child in our community.

A new physical form must be submitted prior to the start of each school year, and it must be updated annually. Physicals are valid for 12 months from the date of the examination, so it is important to plan accordingly to avoid any interruption in your child’s attendance.
